possible, here is what you should be prepared to bring with you to your first
appointment or consultation with one of our doctors:
- Personal identification
- Insurance Information
- List of medications you may be
taking
- Current eyeglasses
- Co-payment for your visit
Also,
for your convenience, you can download a copy of the forms all new patients
are required to complete prior to their eye exam or consultation: Patient
Information & Health History Forms; "Signature on File" form;
"Approved Participants of Care" form; "Notice of Privacy"
as required by HIPPA; and the "Acknowledgement of Receipt," which
